Fissure is nothing but cut in anal canal. Because of the cut in anal canal there is reflux anal spasm. So it becomes a vicious cycle where constipation causes fissure and fissure causes gas, bloating and constipation. So once the fissure is acute it will heal we know but if it is chronic there is a chronic persistent spasm of the muscle and blood supply is completely reduced because it is chronic and deep cut. Lateral Sphincterotomy to release the fissure because it is mainly a surgery which we have to do. There are different ways of doing lateral sphincterectomy. It can be done by a laser or a cautery machine. Other than that we can continue doing conservative treatment or treat by ointment.
